What to Bring to Your First Appointment
Being prepared helps us focus on what matters most — your recovery. Please bring the following to your first visit:
All Patients
- Government-issued photo ID
- Insurance card(s)
- List of current medications
- Any imaging or test results (X-rays, MRIs, etc.)
- Completed intake forms (if sent in advance)
- Comfortable, loose-fitting clothing
If You Were Referred by a Physician
- Physician referral or prescription for physical therapy
- Referring provider’s name and contact information
- Any relevant medical records or surgical notes
